A
certificate of insurance will be required for your rental with
us. Please be sure it is on file with us prior to your rental.
The certificate must have the following three items correctly and cover
the entire rental term duration. In order to reserve your rental, we require
complete insurance information to be received no later than three days
prior to pick up/delivery. We can not be accountable for delays
or refusing a rental if the certificate is late.

The limit of liability shall be $1,000,000.00 combined single limit per
occurrence. A general aggregate limit of at least $2,000,000.00 A
products/completed operations aggregate of at least $1,000,000.00
Evidence of Property Insurance "All risk replacement coverage"
Coverage includes all listed equipment requested, and shall not be for
less than the full replacement value of all equipment rented.
Effective ______________, it is understood and agreed that Fantasee
Lighting, Inc. is recognized as "Loss Payee" and that this policy is
primary and not in excess of, or contributory to, any other insurance
provided for the benefit of, or by Fantasee Lighting, Inc.
Lessee is responsible for payment of the deductible and all associated
costs in collecting the replacement value of the equipment.
